Behaviour Support
Behaviour Support
Our behaviour support practitioners work collaboratively with participants, their family, carers and other support networks to improve overall quality of life and help identify and reduce behaviours of concerns while increasing positive behaviours.
Although our small team is passionate about supporting and understanding all individuals who display behaviours of concern, we often focus on and specialise in providing support in the following areas:
- Providing support to young adults aged 17+ all the way through to the ageing population and elderly aged 65+. The team is passionate about supporting the ageing population and help manage ageing related conditions, such as Major Neurocognitive Disorder (formerly known as Dementia) and/or cognitive decline.
- We specialise in assessing and managing behaviours of concern which may include, but not limited to, aggression (verbal and/or physical), inappropriate and problematic sexualised behaviours, substance misuse or other addictive like behaviours, absconding, social withdrawal or isolation, as well as supporting those with sensory difficulties, and those who have non-verbal communication methods.
- The team is passionate about supporting and responding to those who may experience psychosis (i.e., delusions, hallucinations) and display behaviours of concern as a result.
Darrah Health respect and give thanks to all those who have served in our defence force. We acknowledge the unique nature of military service and sacrifice demanded of all who commit to defend our nation. We have a personal commitment to welcome, embrace and provide support to all military Veterans and their families.
Darrah Health is registered and assessed against Module 2A and is therefore able to provide Intensive and Complex Behaviour Support to participants if needed as per the NDIS Guidelines. This may be implemented with participants who display frequent behaviours of concern that have a significant effect on their or others’ wellbeing and safety, require intensive support and implementation of intensive proactive strategies, and response strategies, which may include the use of restrictive practices to minimise the risk of harm to themselves or others. We are passionate about providing support in the least restrictive manner while maintaining safety to participants and those around them. We therefore specialise in assessing, identifying, monitoring and eliminating the use of Regulated Restrictive Practices (RRP).
Our team has extensive experience in understanding and supporting individuals with complex care needs and dual diagnosis. This can include, but not limited to:
- Traumatic Brain Injuries
- Schizophrenia and other psychotic disorders
- Intellectual Disability and/or other learning disorders
- Autism Spectrum Disorder
- Major Neurocognitive Disorder (Dementia)
- Drug and Alcohol abuse disorders
- Other psychosocial disabilities
- Those with physical health concerns and mobility difficulties
Our team has extensive experience in supporting those living in Supported Independent Living (SIL) homes or Specialised Disability Accomodations (SDA).
Darrah Health is experienced with complex case coordination and is dedicated to work collaboratively. We have a large network and work closely with agencies such as Department of Communities and Justice, Department of Health, Police, NSW Trustee and Guardian, other and allied healthcare professionals, and other non-government agencies.
We are committed to training, supporting and responding to carer and staff needs. Our team is highly trained in providing risk assessments and providing safety intervention training (formerly known as MAPA), as well as the prevention and management of disruptive and destructive like behaviours.
Please do not hesitate to reach out to our team if you wish to discuss your needs further to see if we can support you.
Support Coordination
Support Coordination
Make the best use of your NDIS funding with the help of one of our Coordinator of Supports (COS).
Our team can help participants to find mainstream and other disability services that are right for them so they can maintain their independence, live the life they choose and do the things they enjoy doing.
How can our support coordinators help?
- Assist in understanding what supports are funded in your NDIS plan and how supports can be implemented.
- Work with you to develop a comprehensive plan, assist with identifying your personal goals and ensure you are at the centre of all agreed goals.
- Collaborate with all services involved in your care. This may include your GP, allied health professional, housing provider and other NDIS services.
- Support you through your NDIS plan review process.
- Involve you in all aspects of your support, plan allocation and plan reviews.
Specialist Support Coordination
Specialist Support Coordination
Our specialist support coordinators can help participants with multiple and complex needs overcome barriers, identify and implement solutions and access the right supports.
If a participant has specific environmental or behaviour challenges and requires a higher level of support, our specialist support coordinators can connect them within a specialised support framework and build capacity.
How can our specialist support coordinators help?
- Work with participants to identify and understand the barriers impacting their ability to access, use and maintain their supports.
- Work with participants to find a solution to these barriers and establish a process to identify which need to be addressed first and what supports can be put in place to manage their complex situation and minimise the risk they face.
- Develop a service plan for complex or multiple needs if required, ensure all services can work together, establish communication and collaboration processes, help resolve complex situations and issues, and ensure consistent service delivery.
